Request for Return Material Authorization (RMA) Number

Customers are not authorized to return parts without prior approval from InnowaveRF, LLC. Unauthorized returns will be rejected. There are no other means of requesting a RMA number except completing the form below. Upon approval, a RMA number is issued, via email, along with complete shipping and labeling instructions. Items returned without RMA number will be rejected and the shipment will be returned.

Your RMA will only be processed if it meets the following criteria:

  • RMA request from must be filled out completely.
  • Parts being returned must match original Purchase/Sales order.
  • Copy of purchase order must accompany unit in shipment. Parts must be within warranty period.
  • RMA number must be visible on outside of package.
  • RMA request form must accompany unit for repair.
  • Parts must be returned in adequate condition in proper packaging and shipping material; Failure to do so will result in product returned to sender. If you need immediate replacement, provide InnowaveRF, LLC with a new PO. Please note, InnowaveRF, LLC reserves the right to return product which is deemed customer damaged or no fault found from the RMA.
  • Standard turn around time is 30 days. Expedited services are available for an additional fee.
